New York (CNNMoney.com) — Toyota announced Wednesday night that it is adding another 1.1 million vehicles to an an earlier 4.2 million vehicle recall originally announced in November.
The vehicles are being recalled to fix a problem in which the gas pedal can become caught on the edge of the removable floormat, causing the vehicle to accelerate uncontrollably.

NEW YORK (CNNMoney.com) — About 635,000 cribs sold at major discounters nationwide have been recalled by Dorel Asia SRL for strangulation and suffocation hazards that killed one child and injured 10 others, the government said Tuesday.
The U.S. Consumer Product Safety Commission said that the hardware holding up the side of the crib can fail, causing the drop side of the crib to detach, which could trap and suffocate a child.

NEW YORK (CNNMoney.com) — The maker of several over-the-counter drugs, including Tylenol, Motrin and Benadryl, announced a broad-based recall of these and other drugs Friday after receiving complaints of an “unusual moldy, musty or mildew-like” odor.
The recall drew the FDA’s wrath on Johnson and Johnson (JNJ, Fortune 500) for not reacting quickly to customer complaints and its failure to fix the problem. The company has struggled to resolve the issue since it was first reported in 2008.

Washington (CNN) — A beef recall is under way in a half-dozen states involving possibly contaminated products from the Oklahoma company National Steak and Poultry, according to the firm and federal inspectors.
U.S. Agriculture Department officials said a cluster of illnesses involving the E. coli bacteria was reported in Colorado, Iowa, Kansas, Michigan, South Dakota and Washington state.
The cases then were linked with beef the Owasso, Oklahoma, company produced in October, prompting the government to direct a Class I recall, indicating the highest risk of illness if the products are consumed.
On Thursday, National Steak and Poultry began a voluntary recall of 248,000 pounds of beef products marketed under its name as well as under names that include Carino’s Boneless Beef and Moe’s Beef Steak.

(CNN) — About 700,000 units of Vicks Dayquil cold medicine were recalled Friday after a finding that the packaging was not child-proof, according to the U.S. Consumer Product Safety Commission.
The recall affects Vicks Dayquil Cold & Flu 24-Count Bonus Pack Liquicaps with the UPC no. 3 23900 01087 1, according to the product safety group. The medicine was sold between September 2008 and December 2009.
The packaging — which is not child-resistant and lacks the statement, “This package for households without young children” — should be kept out of the reach of young children. If a child swallows several of the pills, the child could suffer “serious health problems or death,” the commission said.

NEW YORK (CNNMoney.com) — The maker of Slim-Fast announced a recall of its canned, ready-to-drink products due to possible bacterial contamination.
Unilever, the company which also makes products like Skippy peanut butter and Ragu pasta sauce, said the weight-loss drink may be tainted by Bacillus cereus, which can cause diarrhea, nausea and vomiting.
U.S. Food and Drug Administration spokeswoman Rita Chappelle said the recall affects 10 million cans distributed in the United States.

London, England (CNN) — Procter & Gamble is recalling Vicks Sinex nasal spray in the United States, Britain and Germany after finding it contained bacteria, the company said.
Procter & Gamble said it announced the voluntary recall after finding the bacteria in a small amount of product made at a plant in Germany.
There have been no reports of illness from the bacteria, but it could cause serious infections for people with weakened immune systems or those with chronic lung conditions such as cystic fibrosis, Procter & Gamble said late Thursday.
The bacteria poses little risk to healthy people, the company said.

NEW YORK (CNN) — A voluntary recall was announced Monday for more than half a million pounds of ground beef because it may be contaminated with bacteria linked to at least two deaths, officials said.
Fairbank Farms of Ashville, New York, said the recall was issued Saturday for approximately 545,699 pounds of ground beef produced between September 14 and September 16 after the meat was “possibly linked” to E. coli O157:H7.
“Any customers from northeastern or mid-Atlantic states who have identified recalled ground beef products should remove the product from their freezers and return it to their stores for full reimbursement,” the company said.

Penumbra is initiating a voluntary Field Removal of the original version of Neuron 6F 070 Delivery Catheter. For these catheters, Penumbra has received feedback from some users that the catheter could kink or ovalize in certain anatomical situations. This kinking and/or ovalization of the catheter lumen can lead to difficulty in catheter advancement and/or delivery of other devices through the guide catheter. Continued usage of a kinked catheter could cause the catheter to break, which could result in a portion being retained in the patient, which could result in additional procedural time, patient injury or death.
Consumers who have the older version of 6F Neuron 070 Delivery Catheter which is being recalled should stop using and return the device. If your facility is in possession of units with Lot numbers F14630 or higher, you may continue to use these units. Units with Lot numbers F14630 or higher are not affected by this Field Removal Action.

TORRANCE, California — Honda has announced that it is expanding an existing vehicle recall to include 440,000 vehicles to replace a defective driver’s steering-wheel-mounted airbag inflator. The recall includes 2001-’02 Honda Accords, 2001 Honda Civics and 2002-’03 Acura TLs.
“In some vehicles, airbag inflators can produce overpressurization of the driver’s airbag inflator mechanism during airbag deployment,” said Honda in a statement. “If an affected inflator deploys, the increased internal pressure may cause the inflator casing to rupture. Metal fragments could pass through the cloth airbag cushion material, possibly causing an injury or fatality to vehicle occupants
